Procedures for partial changes and for full reconstruction of records
The following are the steps to be observed when requesting for partial changes or full reconstruction of entries in our canonical books. Request can be obtained from the Chancery.
- For Partial Changes, the following are to be secured from the parish.
A. Request for Partial Changes of Parish Records
This is a letter from the parish priest addressed to the Ordinary stating the request to make partial changes in the baptismal and confirmation record.B. Affidavit for Partial Changes of the Parish Record
This is an affidavit stating the purpose and the corrections. This will be executed by the petitioner and after which, signed by the Parish Priest. - For Full Reconstruction of Parish Records, the following are to be secured from the parish.
A. Request Full Reconstruction of Parish Records
This is a letter from the parish priest addressed to the Ordinary stating the request to make the Full Reconstruction in the baptismal, confirmation or marriage record.B. Affidavit for Full Reconstruction of Parish Record
This is an affidavit stating the purpose of the request. This will be executed by the petitioner and after which, signed by the Parish Priest. - The mentioned documents are to be submitted to the Chancery for EVALUATION. Although the initial evaluation of documents will be done at the parish, the final evaluation will be at the Chancery. Together with the said documents are the following important basic requirements:
- A copy of the newly issued birth certificate
- A copy of the baptismal, confirmation, and marriage certificates to be corrected
- Based on the EVALUATION, the need for more supporting documents may or may not be required.
- If there are no problems in the application, a written decree will be issued by the Local Ordinary authorizing the parish priest to grant said request.
- The written decree and the attached documents will now be returned to the parish to allow the implementation of the correction and the issuance of a corrected or a reconstructed certificate.
- Only the persons concerned or their parents may apply for the changes.
- All required documents are valid only for six months from the date of issuance.
